Buildings and Grounds — Based on observation, the indoor play area near the outside play area had several white, mineral fiber ceiling planks that are broken, the ceiling fan is broken, the walls are unclean, there is smoke detector hanging from the ceiling, a broken fire alarm with exposed wiring on the wall near the carbon monoxide detector. In the school-age room, there is a broken arcade machine and a bathroom stall that is not open for use. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision — Based on interviews, it was confirmed there was inadequate supervision resulting in inappropriate interaction between children in care.
Personal Rights — Based on interviews, it was revealed that staff talked and interacted with children inappropriately, which poses a potential health, safety, and personal rights risk to person in care.
Buildings and Grounds — During inspection of school age classroom, LPA observed a large rug that is ripped in several areas. LPA also obseved a chair with white cushions that was stained. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
