Mandated reporter training — Based on LPA records review, the licensee did not comply with the section cited above. S4's Mandated Reporter Training (MRT) expired 10/31/2025 and S11 has NO MRT in file which poses/posed a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on LPA and LPM observations, the licensee did not comply with the section cited above. In the infant classroom - 6 damaged ceiling tiles, P2 - 3 damaged ceiling tiles and in P3 - 4 damaged ceiling tiles. In addition, LPA observed the colored walls in each classroom with paint chippings which poses/posed a potential health, safety or personal rights risk to persons in care.

Drinking Water — Based on LPA and LPM observations and interviews, the licensee did not comply with the section cited above. LPA and LPM observed in the classrooms and outdoors, children’s water bottles are stored in gray containers; however, the bottles were tipped over, waterspouts are touching one another and the containers are collecting dripped water from the bottles which poses/posed a potential health, safety or personal rights risk to persons in care.

Outdoor Activity Space — Based on LPAs interviews, observations, records review and records obtained, the license did not comply with the section cited above. Licensee did NOT ensure the outdoor play area was free of hazard resulting in child 07 sustaining an injury which poses/posed a potential health, safety or personal rights risk to persons in care.

Food Service — Based on LPAs interviews and records obtained, the licensee did not comply with the section cited above. On 06/23/2025, preschool classrooms were served undercooked hamburger patties which poses/posed a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on LPAs observations and interviews, the license did not comply with the section cited above. On 07/01/2025, LPA observed children napping in cots blocking exit doors which poses/posed a potential health, safety or personal rights risk to persons in care.

(b) The licensee may use teacher aides in a teacher-child ratio of one teacher and one aide for every 15 children in attendance. (1) A ratio of one fully qualified teacher (as specified in Section 101216.1(c)) and one aide for every 18 children in attendance in a preschool program is allowed when the aide meets the qualifications specified in Section 101216.2(d). This requierement was not met as evidenced by: Based on LPA observations, interviews and records review P3 had 1 teacher and 1 assistant with 21 children. This poses a potential risk to the health, safety, and personal rights of children in care.

Disaster and Mass Casualty Plan — Based on observations, records review and interviews. The facility failed to post their Disaster Plan in a prominent area were staff would be able to access it as it was stored at an off-site location. Furethermore, interviews disclosed that the facility did not conduct fire drills and did not instruct staff in their duties and responsibilities under the plan, which poses/posed a potential health, safety or personal rights risk to persons in care.

Daily Inspection for Illness — Based on LPA’s interviews and records obtained, the licensee did not comply with the section cited above staff accepts children with signs of illness into care in which poses/posed a potential health, safety or personal rights risk to persons in care.

Health-Related Services — Based on LPAs interviews, the licensee did not comply with the section cited above in 5 out of 7 staff members stated the License does not ensure first-aid supplies are accessible to staff in which poses/posed a potential health, safety or personal rights risk to persons in care.

Reporting Requirements — Based on that the facility did not report incidents to CCL as stated in LIC809 within 24 hours of the occurance. This poses a potential risk to the health, safety, or personal rights of children in care.

Buildings and Grounds — , LPA was informed through interviews that staff did not ensure the classroom was free from choking hazards, due to child choking on the strap due to strangulation. This posses a potential risk to the health, safety and/or personal rights of children in care.
