Personal Rights — Based on evidence gathered and review of video clip of 04/24/25 of Room D2 during nap time where staff(S1&S2) are present, with Licensee Representative and Director throughout the investigation, staff(S1) handled day care child(C1) in a rough manner, which poses an immediate risk to the health, safety and personal rights of children in care.

Personal Rights — LPA received two statements from assistant director and one teacher (S2) stating (S3) engaged in a verbal altercation with S2 and S4 on the toddler playground in front of children on 05/04/2026. The altercation included inappropriate comments about relationship with the parents. A termination letter was also received stating S3 engaged in unprofessional and inappropriate behavior during a workplace incident in the presence of children and staff, which did not align with expected professional standards. This poses a potential risk Health, Safety Personal Rights risk to children in care.

Infant Care Aide Qualifications and Duties — LPA observed S1 who is an aide alone in the classroom without direct supervision of a director or teacher with two infants. This poses a potential risk Health, Safety Personal Rights risk to children in care.

Food Service — Based on observation, interview, record review, the licensee did not comply with the section cited above. LPA observed the food menu is not being followed. Menu stated today, 03/02/2026 brocoli and fruit will be served. LPA observed mixed vegetable and no fruit being served. Not until LPA addressed the issue, Director Hong Ni went to the kitchen to get cuties for the classrooms which poses/posed a potential health, safety or personal rights risk to persons in care.

Food Service — LPA observed facility is not providing the quantity necessary meeting the needs of the children. The facility did not provide fruit during lunch time in the month of February. On 02/02/2026 and 02/12/2026, the facility did not provide a fruit and/or vegetable as required. This poses a potential risk Health, Safety Personal Rights risk to children in care.

Responsibility for Providing Care and Supervision — Based on interviews and evidence gathered, LPA reviewed and observed in the video clip of 04/28/25 and 04/24/25 classroom D2 with Director and Licensee representative that the children(C1, C2 &C3) in video clip of 04/28/25 and a few children in video clip of 04/24/25, care and supervision as necessary to meet the children's needs was not provided, which poses a potential risk to the health, safety, and personal rights of children in care.

Criminal Record Clearance — Based on records review and interviews, Licensee allowed a staff(S1)to work at the facility without criminal record clearances since 01/22/2025, which poses a potential risk to the health, safety, and personal rights of children in care.
