Criminal Record Clearance — Based on record review Staff 1 does not have a completed criminal record clearance and was observed providing care for children during today's inspection. This is an immediate threat to the safety of children in care.

Current roster of children — Based on record review the facility roster LIC 9040 was not filled out completely. Information was missing. This poses a potential risk for the safety of children in care.

Child's Records — Based on record review only 4 out of 6 children present had a file but they were not complete and 2 out of 6 children present did not have a file available. This is a potential safety risk to the children in care

At Least one staff member who is trained in pediatric cardiopulmonary resuscitation and pediatric first aid pursuant to health and safety code 1596.866 shall be present when children are at the child care center or offsite for center activities. Based on record review 3 of 4 staff did not have a valid Pediatric CPR and First Aid approved by Emergency Medical Services Authority EMSA which poses a potential health, safety or personal rights risk to persons in care.

Outdoor Activity Space — Based on observation, interview and record review, the facility did not remove exposed screws from a piece of wood painted yellow in the outdoor play area which poses and potential Health, safety and/or personal rights risk to persons in care.

Drinking water lead testing — Based on interview, the Director did not did not have drinking water tested for lead which poses an potential Health, safety and/or personal rights risk to persons in care.

Buildings and Grounds — Based on observation facility staff did not keep outdoor play area safe by allowing children to play on top of green turf cover which poses a potential Health, safety and/or personal rights risk to persons in care.

Buildings and Grounds — Based on observation facility staff did not making cleaning solutions inaccessible to children by storing Clorox wipes on a wooden cabinet in the children's classroom, which poses a potential Health, safety and/or personal rights risk to persons in care.

Outdoor Activity Space — Based on observation, the facility staff did not maintain the outdoor activity space to be free of hazards. LPAs observed exposed nails, sharp and pointy edges on plastice equipment, and a loose pole on the chain link fence. This poses a did potential Health, safety and/or personal rights risk to persons in care.

Personnel Requirements — Based on observation licensing staff did not have health screening report in personnel file which poses a potential Health, safety and/or personal rights risk to persons in care.

Food Service — Based on observation the facility staff stored disinfectant wipes on the counter in the kitchen area next to the water filter jug. This poses a potential Health, safety and/or personal rights risk to persons in care.

Teacher Qualifications and Duties — Based on observation and interview Director did not provide a photocopy of teacher's transcripts which poses and potential Health, safety and/or personal rights risk to persons in care.

Personnel Records — Based on observation Director did not have Tuberculosis test documents on file for employees which poses a potential Health, safety and/or personal rights risk to persons in care.
