Carbon monoxide detectors required — Based on observation, the licensee did not comply with the section cited above by not having Carbon Monoxide Detectors in the facility, which poses a potential health, safety or personal rights risk to persons in care.

Drinking water lead testing — Based on interview, the licensee did not comply with the section cited above. Founder stated they completed the required lead testing but cannot find her documents. This poses/posed a potential health, safety or personal rights risk to persons in care.
Criminal Record Clearance — Based on observation, interview, record review the licensee did not comply with the section cited above. LPA observed staff Taguhi Harutyunyan and Anastaiya Kimachenko who was fingerprint cleared but not associated with the facility. This poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on record review, the licensee did not comply with the section cited above. Director was not able to provide a EMSA approved pediatric CPR/1st aid certificare for a staff member in the Preschool program, which poses/posed a potential health, safety or personal rights risk to persons in care.
