Limitations on Capacity and Ambulatory Status — Based on LPA's observation and sign in sheet, the facility had 33 children which is beyond the cpacity limit of 30 children.

Responsibility for Providing Care and Supervision — Based on observations and interviews, hildren were left unsupervised during nap time while staff escorted 1 child to the restroom. This poses an immediate risk to the health and safety of children in care.

Drinking water lead testing — Based on record review, the licensee did not comply with the section cited above in that there is no record of lead testing which poses/posed a potential health, safety or personal rights risk to persons in care.

Disaster and Mass Casualty Plan — Based on record review, the licensee did not comply with the section cited above in there was no record of drills being conducted which poses/posed a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above in 3 cans of poisons (raid and hot shot) was not locked away which poses/posed a potential health, safety or personal rights risk to persons in care.

Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above in that outside playground turf has 2 feet x 2.5 deep hole around the stars which poses/posed a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in that 3 of 6 files reviewed were missing mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in that 2 of 6 staff missing tdap, MMR, 6 of 6 missing flu/ flu declination which poses/posed a potential health, safety or personal rights risk to persons in care.

Teacher Qualifications and Duties — Based on record review, the licensee did not comply with the section cited above in that 2 of 6 staff missing record of education which poses/posed a potential health, safety or personal rights risk to persons in care.

Food Service — Based on observation the licensee did not comply with the section cited above in that 20 lb bag rice was observed stored in empty 33 lb laundry container which poses/posed a potential health, safety or personal rights risk to persons in care.

Criminal Record Clearance — Based on record review, the licensee did not ensure S6, date of hire 3/3/26, was associated to the facility which poses a potenial health, safety, or personal rights risk to persons in care. A civil penalty of $500 has been assessed.
