A child was initially accounted for then child unexpectedly separated from the group and fortunately left with his father without any knowledge from any staff.

Personal Rights — Based on interviews and a review of records, the center did not comply with the cited section above. C1’s teacher suggested that, since C1 was reluctant to stay in class, they could leave the classroom. This suggestion led C1 to leave the classroom on their own, which poses an immediate health and safety risk to children in care.

Responsibility for Providing Care and Supervision — Based on the reviewed records and the interviews conducted, the center did not comply with the section cited above. The staff failed to follow the center's sign-in and sign-out protocol, which resulted in a C1 being left at Venitia School and not being picked up. This was uncovered when Venitia School contacted the family, and the center realized the child was not accounted for. Fortunately, the child was still with Venitia Valley School. This situation poses a potential health and safety risk to the children in care. • Following the incident, the center’s director met with all staff members, reviewed active supervision practices, and implemented a new transition protocol. As a corrective action, the center will submit active supervision training materials, including details of the new implementation and procedures, along with the sign-in attendance of the staff members who participated, to Licensing by September 10, 2025, or earlier, to resolve the deficiency.

(d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (C) Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. Based on interviews and record reviews the center did not comply with the cited section above. CCLD was not informed of an unusual incident ocurring.

(1)No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This poses a potential health, safety, or personal rights risk to children in care.
