Family home — in-ground pool drowning prevention requirements — Based on LPA observations and licensee interview, the licensee did not comply with the section cited above. LPA observed an 8x16 gap in the middle of the mesh fence located facing east. In addition, licensee provided LPA with her daily pool inspection log. LPA observed last documented date was 10/4/2025 which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on LPA records review and licensee interview, the licensee did not comply with the section cited above. C1's last 15-minute sleep log was documented for 05/06/2026 and C2 who has been enrolled since 05/26/2026 does not have 15 minute sleep logs which poses/posed a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on record review, the licensee did not comply with the section cited above. LPAs observed the most current fire and disaster drills were conducted on 07/19/2022 by Licensee. Licensee failed to conduct the fire and disaster drills at least once every six months. This poses a potential health, safety, or personal rights risk to children in care.
Immunizations — Record reviews of the four children’s file, three children’s file (Child #2, Child #3, and Child #4) did not have proof that they have been immunized against diseases as required by the California Code of Regulations, Title 17, beginning with Section 6000. This poses a potential health, safety, or personal rights risk to children in care.
Child's Records — Based on record review, the licensee did not comply with the section cited above. The following forms are not in Child #2 (C2), Child #3 (C3), and Child #4's file: LIC 282, LIC 700, LIC 627, LIC 995A, and LIC 9150.
