Responsibility for Providing Care and Supervision — Based on interview, record review, and observation staff completed face to name check and C1 went to the restroom without staff knowledge. The staff took the other children outside and C1 was unaware the class left and was alone inside the classroom for five minutes without staff supervision which is a potential health, safety, or personal rights risk to the persons in care.

Responsibility for Providing Care and Supervision — Based on interview, observation, and record review, C1 was playing on the play structure monkey bars when they fell off the bars into the woodchips. Staff S1 & S2 were not in close proximity, they did not have visual observation of C1 and did not witness C1 fall. S1 & S2 learned of the injury after C2 & C3 brought C1 to them which is a potential health, safety, and personal rights risk to the persons in care.

Buildings and Grounds — Based on observation, interview, the licensee did not comply with the section cited above in the carpet inside the school aged class was torn and ripped as a potential tripping hazard which poses a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation, interview, the licensee did not comply with the section cited above in a lower cabinet was observed in the Kindergarten classroom unlocked with borax powder and alka seltzer as chemicals which poses a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on interviews and records, it was determined that C1 was left unsupervised in the outside play yard of the facility for an approximately one minute. This poses a potential risk to the health, safety or personal rights of the children in care.
