The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. — Based on interview and observation, the licensee did not comply with the section cited above, there are toy, equipment, and shelves that show signs of collection of dirt or spilled food, which poses a potential risk to the health, safety of persons in care.
Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above in five out of six children's records were missing the Physicians Report (LIC701) which poses a potential health, safety risk to persons in care.
