Responsibility for Providing Care and Supervision — Based on an unusual incident report and director statement, on 4/17/26, a child left the facility playground without staff being aware at 10:30 a.m. and was found by a teacher from the elementary school that the facility is located at, at 10:34 a.m. which poses a potential health, safety or personal rights risk to persons in care.
(3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. Responsibility for Providing Care and Supervision — Based on two Unusual Incident Reports submitted by the facility and interviews from staff, two children (C1, C2), eloped from the playground into the Calpella Elementary School playground without being observed which poses a potential health, safety or personal rights risk to persons in care.
No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. — Based on an Unusual Incident Report submitted by the facility and interviews from staff, a child (C1), eloped from the playground into the parking lot during a time where moving cars were present which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on record review and interview, the facility did not possess immunization records for S1-S3 which poses a potential health, safety or personal rights risk to persons in care.
