Criminal background check — Based on observation, interview and record review, the licensee did not comply with the section cited above. Licensee allowed Adult #1 (A1) to provide care and supervision to daycare children for two weeks. A review of records, including the Guardian Employee Roster, shows that A1 does not have a criminal record clearance which poses an immediate health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on observation and interview, the licensee did not comply with the section cited above. Licensee has three infants in care and one play yard. LPA observed the infants in care would not be able to climb out of a crib or play yard which poses a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above. LPA observed Staff #1 (S1) was missing the required training, as well as the LIC 9108 Statement Acknowledging Requirement to Report Child Abuse, LIC 9052 Employee Rights, documentation for required immunizations, and a TB clearance/ test which poses a potential health, safety or personal rights risk to persons in care.

Immunizations — Based on record review, the licensee did not comply with the section cited above. A review of records show three out of five children's files reviewed were missing immunization records. LPA was unable to verify if the children were immunized prior to admission to the daycare which poses a potential health, safety or personal rights risk to persons in care.

Family home — home free of safety hazards — Based on record review, the licensee did not comply with the section cited above. A review of records showed two infants in care were missing an emergency information card and consent for emergency medical treatment. One infant did not have any documentation at all which poses a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above. Licensee does not have 15 minute sleep logs for three infants in care for the majority of February 2026 and all of March 2026 which poses a potential health, safety or personal rights risk to persons in care.

Information provided to parents on accepting a child — Based on record review, the licensee did not comply with the section cited above. Licensee has not provided parents of newly enrolled chilldren with the Annual Report report dated 03/11/2025 that documents a Type A citation which poses a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above. A review of records show Licensee completed Mandated Reporter Training on 01/29/2023 and expired on 01/29/2025 which poses a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on interview and record review, the licensee did not comply with the section cited above. LPA observed Licensee has not kept a record of 15 minute sleep checks for two infants in care. Licensee stated she was unaware that 15 min sleep checks were required until the infants turn 24 months which poses a potential health, safety or personal rights risk to persons in care.

Family home — home free of safety hazards — Based on observation, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.
