Staff S1 did not conduct a headcount during the transition from outdoor to indoor activities, resulting in C1 being left outside unattended.
Responsibility for Providing Care and Supervision — On 1/30/2025 staff (S-1) was observed by staff (S-2) leaving a child unsupervised outside the door that leads to the playground. Staff (S-1) and the other children entered the classroom and the door was closed, leaving child (C-1) unspervised outside. Staff (S-2) immediately told staff (S-1) that child (C-1) was outside and redirected child back inside classroom. This poses an immediate threat to the health and safety of children in care.

Drinking water lead testing — Based on interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Director was unable to provide the lead test result/documents.
Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed one fence in the garden area is unstable.
