Family home — home free of safety hazards — Based on observation, interview and record review the licensee did not comply with the section cited above in that there were cleaning compounds such as bottles of Lysol spray and Lysol bottles, Pinol and other cleaning compounds on the kitchen counter next to the sink which posed a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on observation and interview the licensee did not comply with the section cited above in that around 10:53am when LPA observed C2 sleeping on a car seat that was on top of the dinning table which poses a potential health, safety or personal rights risk to persons in care.
Family home — preventive health training, including pediatric CPR and first aid — Based on observation, interview and record review the licensee did not comply with the section cited above in that licensee and S2 did not have an updated CPR completed. LPA observed CPR expired on April 2025 for both adults which poses a potential health, safety or personal rights risk to persons in care.
Family home — admission procedures and parent rights — Based on observation, interview and record review the licensee did not comply with the section cited above in that C2, C3, C4 and C5 did not have LIC 995A completed and signed in children files which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, interview and record review, the licensee did not comply with the section cited above in that C2 did not have any 15min. sleep logs completed nor documented since child started receiving services which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above. Licensee's mandated reporter training expired 8/19/2024 which poses a potential health, safety or personal rights risk to persons in care.
