102425 INFANT SAFE SLEEP (a) There shall be one crib or play yard for each infant who is unable to climb out of the crib or play yard. 3) Mattresses shall be firm and covered with a fitted sheet that is appropriate to the mattress size, fits tightly on the mattress, and overlaps the underside of the mattress so it cannot be dislodged. (i) If an infant falls asleep before being placed in a crib or play yard, the provider shall move the infant to a crib or play yard as soon as possible. This requirements was not met as evidence by; LPAs observed crib with a sunken in mattress with no fitted sheet. Licensee stated that mother will be bringing a pack and play and she holds the infant in the snuggli when the infant naps. This is a violation that if not corrected poses an immediate risk to the health and safety of the children in care.

Staffing Ratio and Capacity — On today's inspection LPA observed 12 children present with Licensee and Licensee's assistant-Facility license maximum capacity at one time is 8 children. This is a requirement that if not corrected poses an immediate health and safety risk to children in care.

Criminal Record Clearance — ; LPA observed two unfingerprinted adults assisting with the children on today's inspection. This is a requirement that if not corrected poses an immediate health and safety to the children in care.

Staffing Ratio and Capacity — On today's inspection LPA observed 10 children present (including 2 of Licensee's own children). 102417 Operation of a Family Child Care Home (b) The home shall be kept clean and orderly, with heating and ventilation for safety and comfort. The requirement was not met as evidence by; LPAs observed the activity area in the home to be cluttered with Licensee's laundry, making most areas in the space inaccessible for children. This is a requirments that if not corrected can pose a risk to the health and safety of the children in care.

Family home — home free of safety hazards — Based on observation, the licensee did not comply with the section cited above by ghaving clening compounds acessible to the children which poses a potential health, safety or personal rights risk to persons in care.

Staff immunization — influenza, pertussis and measles — Based on record review a staff #1 did not have proof of TB test which poses/posed a potential health, safety or personal rights risk to persons in care.

Family home — clean, safe, heated and ventilated — ; LPA observed dog feces in the backyard accessible to the children. This is a deficiency that if not corrected can pose a risk to the health and safety to the children in care.

102417 Operation of a Family Child Care Home (g) The home shall be free from defects or conditions which might endanger a child. This requirements was not met as evidence by; LPA observed the fence has large broken pieces of wood. This is a requirements that if not corrected can pose a risk to the health and safety of the children in care.

Family home — home free of safety hazards — Based on observations, the licensee did not comply with the section cited above by having hazardous items accessible in the children's bathroom which poses a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above by Licensee not having her Mandated Reporter training updated which poses a potential health, safety or personal rights risk to persons in care.
