Immunizations — Based on observation, interview and record review, the licensee did not comply with the section cited above for one child, C3 which poses a potential health, safety or personal rights risk to persons in care. Licensee stated that child may be on a delayed immunization plan. LPA provided Licensee with information on how to obtain a medical exemption and a copy of regulation 102418 (d) which discusses how to admit a child that lacks documentaion of having received all the shots and does not have a permanent medical exemption.
Mandated reporter training — Based on (interview) (record review), the licensee did not comply with the section cited above for one staff (S2) which poses a potential health, safety or personal rights risk to persons in care. Upon file review, LPAs observed staff S2's Mandated Reporter training has expired.
Infant Safe Sleep — Based on observation, interview and record review, the licensee did not comply with the section cited above for two infants (C1 and C3) which poses a potential health, safety or personal rights risk to persons in care. Licensee stated that she had begun conducting 15 minute sleep checks and documenting them but had discontinued to do so. LPA reminded Licensee that all infants under 2 years of age need to be physically checked every 15 minutes. Nap log shall be completed as checks are done on the infant.
