Personnel Records — Based on interviews conducted and record review gathered during complaint investigation, the licensee did not comply with section cited above, as LPA determined personnel records for former staff were not maintained and were not kept for the required three years, which poses a potential health, safety or personal rights risk to persons in care.
102416.3 Alterations to Existing Buildings or Grounds (a) Prior to making alterations or additions to a family child care home or grounds, the licensee shall notify the Department of the proposed changed, including, but not limited to, the following: (6)Any change from an area of the family child care home previously identified as "off limits" to an area where care and supervision will be provided to children in care. This requirement has not been met as is evident: Based on observation on August 14, 2025, LPA observed cribs and children sleeping in the off limits master bedroom, which poses a potential health and safety hazards to person(s) in care.
