Limitations on Capacity and Ambulatory Status — LPAs observed at various inspections at least 8 children to be enrolled and actively attending after they have turned 24 months of age.
Reporting Requirements — LPAs review of staff and childrens files show incident reports of the staff feeding a child food the child is allergic to on multiple occasions which were not reported to the Department.
The infant shall be fed in accordance with the individual plan. The requirement is not met as evidence by Based on interviews and record reviews, the teacher fed an infant food the infant is allergic to on two different days... which poses an potential Health and, Safety and personal Rights risk to persons in care.
Infant Care Activities — Based on observation, interviews conducted and record review, the Center did not comply with the section cited above as staff were not able to meet infant child’s needs, which poses a potential risk to the health, safety, and personal rights of children in care.
Teacher Qualifications and Duties — LPAs spoke to staff present at the time of arrival and were informed that S3,S4 did not have 12 post-secondary units.
Child Care Center Director Qualifications and Duties — When LPAs arrived at 2:00PM Site Supervisor was not present and there was not a fully qualified teacher present to act as a substitute site supervisor. Site Supervisor arrived at 2:51PM
Staff-Infant Ratio — At time of the inspection S3 and S4 were caring for 4 infants, however neither were a fully qualified teacher.
