Family home — home free of safety hazards — Based on observation, the licensee did not comply with the section cited above as LPA observed multiple knives, sharp objects, and other hazards in one of the lower kitchen drawers, in bedroom 2, and in the backyard which was accessible to children in care which poses an immediate health, safety or personal rights risk to persons in care.

Staffing Ratio and Capacity — Based on observation and record review, the licensee did not comply with the section cited above as upon LPA arrival LPA observed seven children present at the facility, four of the seven children were infants and the three remaining children were between the ages of 2 years old and 4 years old, and no children were enrolled in TK or school per Licensee which poses an immediate health, safety or personal rights risk to persons in care.

Alterations to Existing Building or Grounds — Based on observation and interview, the licensee did not comply with the section cited above as LPA observed that the carport and backyard, which were previously off-limits, had evidence of children's toys and Licensee stated that children had played in the carport earlier that morning; furthermore, off-limits master bedroom closet no longer had a door/barrier that prevented children in care from accessing the room which posed a potential health or safety risk to persons in care.

Immunizations — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C1 was missing proof of immunizations which poses a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C1, C2, and C3 were missing proof of 15 minute sleep checks which poses a potential health, safety or personal rights risk to persons in care.
