Staff immunization — influenza, pertussis and measles — Assistant Helen Vargas does not have pertussis immunization on file. This poses a potential risk Health, Safety Personal Rights risk to children in care.

Licensee Marian Casillas states they are in process of separating and he lives in Redwood City which poses/posed a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on record review, the licensee did not comply with the section cited above. Child 1, 3, 7, 8, 9 and 10 immunization records need to be updated which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above. Child 3 is missing LIC9227 which poses/posed a potential health, safety or personal rights risk to persons in care.
