Responsibility for Providing Care and Supervision — Based on interviews, C1 was missing and found unsupervised in the restroom which poses/posed a potential Health and Safety risk to persons in care.
Infant Care Center Fixtures, Furniture, Equipment, and Supplies — Based on observation, the licensee did not comply with the section cited above in that LPA observed staff changing a childs diaper, the sink was behind the staff and they were not able to reach which poses/posed a potential health, safety or personal rights risk to persons in care.
