Responsibility for Providing Care and Supervision — Based on records reviewed and interviews conducted, it was determined that the staff failed to provide adequate supervision resulting in a child falling off from a bench and sustaining a head injury. This poses an immediate risk to the health, safety, and personal rights to the children in care.

Teacher-Child Ratio — Based on interviews in at least one instance the facility has operated out of ratio during transitionings. This Posed a potential risk to the health, safety, and personal rights of children in care.

Napping Equipment — Based on observation and interviews conducted, it was determined that there were children's napping equipment that were in contact with other bedding. This poses a potential risk to the health, safety, and personal rights of children in care.

Personal Rights — Based on interviews conducted and records reviewed it was determined that more than one child had wet clothes until they were picked up. This poses a potential risk to the Health, Safety, and Personal Rights of children in care.

Disaster and Mass Casualty Plan — Based on interview, the licensee did not comply with the section cited above due to the staff being unable to locate the fire drill log which poses a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on staff, parent, and documents reviewed it was determined that there were several instances where children were unsupervised and the incidents were not reported which poses a potential risk to the health, safety and personal rights of children in care.

Admission Policies - (a) Every child care center shall have all admission policies in writing and available to the public. The policies shall coincide with the limitations stated on the license, and shall include, but not be limited to, the following: (5) Field-trip provisions, if any. — Based on interviews and records reviews it was determined that the facility did not maintain signed permission for field trips in the children's files, which is a potential risk to the health, safety, or personal right of children in care.

Child Care Center Director Qualifications and Duties — Based on interview and record review it was determined that this facility has not had a Director since December 31, 2022. This poses a potential risk to the Health, Safety, or Personal Rights of the children in care.
