LPA interviewed the three staff members for ECE units and stated that they don't have complete units. This poses an immediate health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on LPAs observations on this day, there were three instances recorded at 2:22 PM, 3:09 PM and 3:55 PM when children were observed to be outside of the classroom without active visual supervision, which poses an immediate health, safety or personal rights risk to persons in care.

Teacher-Child Ratio — At 2:50 PM, Based on record review, interview, and observation, LPAs confirmed there were 31 school-age children present in Room 1 being supervised by two staff. LPAs observed the facility is operating over capacity.

Personal Rights — Based on the course of the investigation, interviews were conducted and the facility did not comply with section cited above which posed a potential health, safety or personal rights risk to children in care.

Buildings and Grounds — Based on interviews, the facility did not comply with section cited above as staff had observed there to be mice and droppings of mice inside one of the classrooms. This posed a potential health, safety or personal rights risk to children in care.

Buildings and Grounds — Based on site observations, and interviews conducted during the course of this investigation, it was confirmed that a baby gate was installed briefly in the doorway to prevent children from leaving the classroom.

Reporting Requirements — Based on interviews, LPAs confirmed that the program occasionally uses an unlicensed area notably the open area located behind the portables.

Drinking water lead testing — Based on observation and interview with the director, the licensee did not comply with the section cited above by not testing for lead water, which poses a potential health, safety or personal rights risk to persons in care.
