The above regulation was not met as evidence by: in the month of October 2025, C1 was involved in two unusual incidents, one incident C1 got a body limb stuck in a chair, and in the second incident C1 got their head stuck in the same chair. In the second incident fire department arrived at the FCCH, Licensee failed to notify the department.
Family home — home free of safety hazards — Based on (interview) and observations, the licensee did not comply with the section cited above in one required object(pool vocer) which was not in place during inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on (record review)], the licensee did not comply with the section cited above in during LPAs file review, in one (persons) file review. LPA discovered AB1207 certificate expired which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation and interviews Licensee did not comply by having infants sleep in a separate room with the door closed which poses a potential risk to the health, safety and or personal rights of children in care.
Personal Rights — Based on interviews conducted Licensee did not comply since infant hygiene needs were not being met which is a potential risk to the health, safety and or personal rights of children in care.
