Staff immunization requirements — During records review LPA Herrera observed that 4 out of 7 staff members were either missing immunizations records or had incomplete immunization records on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision — staff not supervising preschool children in restrooms and leaving children in back classroom while all staff are in the front classroom, which poses a possible threat the the health and safety or personal rights to children in care.
Mandated reporter training — Based on observation, interview, and record review, the licensee did not comply with the section cited above. 1 staff has not completed the mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.
Sign In and Sign Out — Based on observation, interview, and record review, the licensee did not comply with the section cited above. LPA observed in 2 classrooms 6 parent signatures were missing from the sign in sheet which poses a potential health, safety or personal rights risk to persons in care.
Teacher-Child Ratio — Based on observation, interview, and record review, the licensee did not comply with the section cited above. LPA observed 19 children in Advanced Preschool classroom with 1 teacher and 1 aide. Upon reviewing files LPA could not find any units for the aide. LPA spoke with aide and aide stated she did not have any units so therefore they are over ratio which poses a potential health, safety or personal rights risk to persons in care.
