Personal Rights — interviews. It was determined that licensee is providing care to children at her store in Shafter, CA and also at the home (unknown location) of adult #1 which is not licensed. This poses an immediatel risk to the health, safety and/or personal rights of children in care.
Criminal Record Clearance — Interview. Multiple individuals that were interviewed reported that daycare children have received care by adult #1 during licensees noted daycare hours and in the home of adult #1 who does not have a criminal record clearance on at least three occasions in the last month. This poses an immediate risk to the health, safety and/or personal rights of children in care. A $500.00 civil penalty was assessed today.

Large family home licensing requirements — Based on LPA observation, the licensee did not comply with the section cited above. LPA observed that the home's carbon monoxide detector war not working which poses a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on record review, the licensee did not comply with the section cited above in 1 out of 3 infant files which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above for 3 out of 3 infants which poses a potential health, safety or personal rights risk to persons in care.

Family home — home free of safety hazards — Based on LPA observation, the licensee did not comply with the section cited above. LPA observed accessible laundry room with no door that had cleaning supplies and laundry soap inside which poses a potential health, safety or personal rights risk to persons in care. Licensee installed a baby gate at the time of inspection in order to make the laundry room inaccessible.
Mandated reporter training — Based on records review, the licensee did not comply with the section cited above. Licensee along with 4 other Staff members had expired Mandated Reporter Training Certificates, which poses a potential health, safety or personal rights risk to persons in care.
