Inspection Authority of the Department — Based on interview, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. LPAs observed 13 dropped off with 3 staff members going in to the facility. Upon entering into the facility and during the inspection LPAs were able to verified 2 staff members and 12 children. LPAs requested to inspect the off limit areas and Ms. Chen said she does not have the keys to open the rooms.
Staffing Ratio and Capacity — Based on observation, interview, record review, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. LPAs observed 13 children being dropped off at the facility and observed 12 children inside the facility (2 infants, 10 preschool children). Based on record reviewed provided by Ms. Chen, no TK and/or elementary children records.

Criminal Record Clearance — (1) Based on observation, interviews, and record review, a helper's criminial record clearance did not associate to the facility who was supervising the enrolled children. This poses potential Health and Safety risk to persons in care.
Reporting Requirements — Based on the interviews and record review, conducted during today’s inspection the licensee did not submit unusual incident report to CCL, which poses potential health, safety, or personal rights risk to children in care.
Site visit notice posted at the facility — LPA observed LIC 809, LIC 809-D, and Notice of Site Visit were not posted on the wall which poses a potential health and safety risk to children in care.
Notice to parents that site visit reports are available — Licensee was unable to provide LIC 809, LIC 809-D , and LIC 9224 Acknowledgement of Receipt of Licensing Reports to LPA. Per licensee, not all parents received and signed the LIC 9224 as required for a type A violation which is a potential risk to the health and safety of children in care.
