Responsibility for Providing Care and Supervision — Based on documentation review and interviews that were conducted the facility did not comply with the section cited above, one child was left alone for approximatetly 1 hour & 21 minutes in the PreK1 classroom, which poses/posed an immediate health, safety or personal rights risk to persons in care.
Teacher-Child Ratio — Based on LPA observations, record reviews, and interviews that were conducted the facility did not comply with the section cited above and it was found that the facility has been operating out of ratio. During today's inspection LPA observed 25 children in care with 2 adult staff members providing care and supervision, which poses/posed an immediate health, safety or personal rights risk to persons in care.
