For a license issued before July 1, 2022, initial testing ... and posted before January 1, 2023.Based on record review and interview with the Director, the facility did not complete the required lead testing of its drinking-water by January 1, 2023. This poses a potential risk to the health and safety of children in care.
Disaster and Mass Casualty Plan — Based on record review, the licensee did not comply with the section cited above in that they have not had a emergency drill since 10/25 which poses a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on LPA interviews with relevant parties, multiple staff members have been witnessed and heard yelling at a child in care. This poses a potential health, safety or personal rights risks to the children in care.
Limitations on Capacity and Ambulatory Status — Based on interviews, the school-age room was over capacity for approximately 10 days in the past year. Staff could not recall the days that the room was over capacity.
