Reporting Requirements — Based on interview, Site Director Amber Guttierez was promoted to a Director position sometime in 2024. Licensee failed to report this changed to the department.
Criminal Record Clearance — Based on record review, licensee failed to associate a staff to the facility. It is noted that this staff is a volunteer who works more than 16 hours per week.
