Based on file review, staff interviews, and information gathered during another matter, the facility did not report to the Department about an incident that occurred in August 2025, that involved a staff member (S7) mishandling a child (C1), which poses/posed a potential health, safety, or personal rights risk to children in care.

Staff immunization requirements — Based on records review 2 of 5 staff files (S4, S5) were missing immunizations records and 2 of 5 staff files (S2,S4) were missing LIC 503 Health screenings which poses/posed a potential health, safety or personal rights risk to children in care.
