Mandated reporter training — Based on record review and interview the licensee did not comply with the section cited above in 2 out of 3 staff members did not have current certificaiton, which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in 1 out of 3 persons did not have proof of immunization, which poses/posed a potential health, safety or personal rights risk to persons in care.
