Responsibility for Providing Care and Supervision — Based on staff interviews and staff stating once C1 entered the hallway, C1 was not visible and was unsure if C1 went into the kitchen or not, this is an immediate risk to the health and safety of children in care.

Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above. While touring the facility at 12:51 p.m., LPA observed an unlocked cabinet inside classroom #4 with nine toddler children present. Inside the unlocked cabinet, LPA observed one Sanitizing Bleach Solution and one spray bottle labeled Sanibet that reads "Hazard Warning: Corrosive, Health Hazard: Skin and eye irritant, inhalation irritant, and harmful if ingested" which poses an immediate health, safety or personal rights risk to persons in care.

Criminal Record Clearance — Based on record review and interview, Rosa Reynoso has worked at the facility without clearance transfer since 7/15/2024. The Assistant Director failed to submit transfer request form to licensing. This poses a potential Health and Safety risk to the children in care.

Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above. While touring the outside patio area at 9:45 a.m. LPA observed sharp rotted wood and rotted nails around the window and doors. Assistant director stated preschool children utilize the patio area which poses an immediate health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above. While touring inside of the facility at 8:57 a.m. LPA observed 10 preschool age children present with 1 staff. LPA observed the bathroom door open and inside, LPA observed Peroxide Cleaner that reads "Keep out of reach of children" that was accessible to children in care. Also, while touring the the classrooms #1 and #2, LPA observed a can of Raid Ant Killer inside an unlocked cabinet under the children's water faucet that reads "Keep out of reach of children." Inside classroom #6 there were 15 children with two staff. LPA observed bleach water inside and unlocked cabinet on the lower shelf that children had access to. bleach water bottle reads "corrosive, skin and eye irritant, inhalation irritant, and harmful if ingested" which poses an immediate health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on the UIR submiteed and staff interviews, staff stating they did not see when the incident occured and was only made aware of the incident when C1 informed S1.This is an immediate risk to the health and safety of children in care.

101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time.... Supervision shall include visual observation. This requirement was not met by: Based on the UIR submitted to the Orange County Regional Office, interviews conducted on 2/27/2024 and staff stating C1 was left alone without any adult supervision.

Responsibility for Providing Care and Supervision — Based on staff interviews, staff stating C1 was left inside the classroom alone for approximately 1 to 2 minutes and another staff stating C1 was left alone inside the classroom under 5 minutes but not sure of the actual time.This is an immediate risk to the health and safety of children in care.

Disaster and Mass Casualty Plan — Based on interview and record review, the licensee did not comply with the section cited above. Assistant director was unable to provide LPA documentation showing disaster drills are being conducted and documented at least every six months which poses/posed a potential health, safety or personal rights risk to persons in care.

Notice to parents that site visit reports are available — LPA observed child #2 (C2) did not have a written statement or LIC 9924 inside their file stating the parents were informed of the incident dated 1/26/2024.
