Immunizations — Based on record review, the licensee did not comply with the section cited above. The licensee did not comply with the section cited above. LPA observed four children's Immunization Records were not transcribed onto the blue CDPH 286 form, which poses a potential health, safety or personal rights risk to persons in care.
Child's Records — Based on record review, the licensee did not comply with the section cited above in, reviewed that multiple documents were missing children's authorized representative's signatures which poses a potential health, safety or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision — LPA observing child in an emply classroom on a table at 12:40pm on 2/26/25 without teachers knowledge. This poses a potential health and safety risk to the children in care.
Preschool Program with Toddler Component — Based on LPA's observation three toddler aged children were commingling with the pre-school aged children in the pre-school classroom.
Personal Rights-The licensee shall ensure that each child is accorded the following personal rights...To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature... This requirement was not met based on observation by LPA Ouye observing staff S1 yelling at toddlers which is a personal rights violation of the children in care.
Mandated reporter training — Based on interview with Director & Staff record review, the facility did not comply with the section cited above. Director could not provide proof of completion of the mandated reporter training for all staff, which poses a potential health, safety risk to persons in care.
Staff immunization requirements — Based on record review, the facility did not comply with the section cited above. LPAs reviewed that 2 staff member's proof of immunizion, influenza, pertussis, and measles, were not on file which poses a potential health, safety risk to persons in care.
