Notice to parents that site visit reports are available — Health and Safety Code Section §1596.8595 reports to be provided to parents or guardian of each child receiving services, (c)(1) A licensed child day care facility shall provide to the parents or guardians of each child receiving services in the facility copies of any licensing report that documents any Type A citation that represents an immediate risk to the health, safety, or personal rights of children in care as set forth in paragraph (1) of subdivision (a) of Section 1596.893b. Based on observation and record review of 9 children's files, the licensee did not comply with the section cited above in each of the 9 children's files were missing proof of 1 or more required LIC9224 Acknowledgement of Receipt of Licensing Reports signed receipts, for reports dated 02/01/2024, 06/25/2024 04/05/2023 and 09/07/2022, according to the children's enrollment dates, which poses an immediate health, safety or personal rights risk to persons in care.

101229(a)(1) Responsibility for Providing Care & Supervision (a)The licensee shall provide care & supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time... This regulation was not met as evidenced by: Based on LPA record review & interviews conducted, it was determined that on 11/20/2023 a Lack of Supervision occured where a daycare child was left unattended for 6 minutes by staff assigned to supervise, which posed an immediate risk to the health & safety of children in care.

Mandated reporter training — Based on observation and record review, the licensee did not comply with the section cited above where 1 staff present and caring for children did not renew ther Mandated Reporter training after expiration on 05/2024, which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on observation and record review, the licensee did not comply with the section cited above in 2 staff present and careing for children are missing proof of measles vaccination and 1 staff present is missing proof of pertussus vaccination which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — 101216(f) At least one staff member who is trained in pediatric cardiopulmonary resuscitation and pediatric first aid pursuant to Health and Safety Code Section 1596.866 shall be present when children are at the child care center or offsite for center activities. Based on observation and record review, the licensee did not comply with the section cited above in the facility did not ensure at least 1 staff present to open the facility at 6:30 AM and care for children were trained in pediatric cardiopulmonary resuscitation and pediatric first aid and specified staff CPR/1st Aid certification expired in 06/2024, which poses a potential health, safety or personal rights risk to persons in care.

This regulation was not met as evidenced by: Based on LPA record review, the licensee failed to notify the department of reported incidents that occured during operation hours regarding Lack of Supervision & Personal Rights violations, regarding staff & children, which posed both immediate & potential risks to the health & safety of children in care.
