(1) No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation. Thie requirement was not met as evidenced by a child receiving a bruise on 04/24/2024 while in care which staff are unable to explain how the child obtained the bruise. Teacher-Child Ratio (a) There shall be a ratio of 1 teacher visually observing and supervising no more than 12 children in attendance... This regulation was not met as evidenced by 15 preschool aged children being supervised by 1 staff on 05/20/2025.
