Buildings and Grounds — Interviews and LPA observations revealed that classroom floors and restroom areas were not well maintained. In one classroom, the floor areas around tables were stained, sticky and showed small spills. In the restroom, LPA observed dirt and grime around floor edges and toilet base. Toilet seats showed urine stains and a stained toilet bowel. The metal divider had exposed rust on the bottom edges. In the outdoors, there were several paper cups scattered on the ground.
Teacher-Child Ratio — Staff interviews and record reviews revealed multiple instances of staff being out of ratio while caring for children. This poses a potential risk to the health, safety, or personal rights to children in care.
Personal Rights — Based on interviews conducted and information gathered, children were observed in a chaotic environment, climbing on tables and unanchored play furniture, on multiple occasions while the teacher was changing diapers. This posed a potential risk to the health and safety of the children in care.
Buildings and Grounds — Based on interviews conducted and information gathered, the classroom was messy and unorganized with children’s blankets, personal belongings and other items on the floor throughout the day. This posed a potential risk to the health and safety of the children in care.
Teacher-Child Ratio — It was revealed that staff have been out of ratio on more than one occasion, while coverage is being arranged. This poses a potential risk to the health, safety, or personal rights to children in care.
Indoor Activity Space — Based on observation, the licensee did not comply with the section cited above. A large area rug in Early PS class was stained and ripped along the edge with loose fabric which posesa potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in 9 out of 20 staff did not have a current mandated reporter training on file which poses a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in 9 out of 20 staff did not have a completed health screening on file which poses a potential health, safety or personal rights risk to persons in care.
