Buildings and Grounds — Interviews and LPA observations revealed that classroom floors and restroom areas were not well maintained. In one classroom, the floor areas around tables were stained, sticky and showed small spills. In the restroom, LPA observed dirt and grime around floor edges and toilet base. Toilet seats showed urine stains and a stained toilet bowel. The metal divider had exposed rust on the bottom edges. In the outdoors, there were several paper cups scattered on the ground.
