Based on interview, the licensee did not comply with the section cited above. S1 was unaware of altercation between children. Fixtures, Furniture, Equipment and Supplies — Based on observation, the licensee did not comply with the section cited above. During indoor physical plant inspection, LPA observed classroom C and classroom D, did not have sufficient quantity of variety of age-appropriate equipment, toys and materials. This poses/posed a potential health, safety or personal rights risk to persons in care.
Teacher Qualifications and Duties — Based on record review, the licensee did not comply with the section cited above. Staff #1 is missing 9 units and Staff #4 is missing 6 units in Program/Curriculum. Staff #1 and Staff #4 are not fully qualified teachers and were observed to be in a classroom with 14 children. This poses/posed a potential health, safety or personal rights risk to persons in care.
Health-Related Services — Based on record review and interview, the licensee did not comply with the section cited above. During review of stored medication, LPA observed 6 out of 6 children's medication stored in the medication box that belong to children who no longer attend at the facility and/or no longer need the medication. This poses/posed a potential health, safety or personal rights risk to persons in care.
