Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above in one count out of one count which poses a potential health, safety risk to persons in care. LPA observed the wooden bottom of the fences around playground had splinters and some nails sticking out of the wood. On another playground some dried bushes of tress piled up over the fence to create possible fire hazard.
Personnel Requirements — Based on observation, and record review, the licensee did not comply with the section cited above in 3 counts out of 7 counts of staffs' files missing LIC 503 for Staff # 7 and LIC 503 plus TB test for Staff # 5, and 6. which poses a potential health, safety risk to persons in care.
Napping Equipment — Based on LPA's observation from the classrooms today, the licensee did not comply with the section cited above in 3 counts out of 3 counts of 3 classrooms that the floor mats were not in good condition and the foam were exposed in 3 classrooms which poses a potential health and safety risk to persons in care.
Outdoor Activity Space — Based on observation today, the licensee did not comply with the section cited above in two counts out of two counts that the sandboxes contained dirt and not adequate sand and the sandboxes contained foreign materials in it which poses a potential health, and safety risk to persons in care.
Staff immunization requirements — Based on the record reviewing today, the licensee did not comply with the section cited above in 2 counts out of 5 files that staff # 2 missing MMR, and staff # 3 missing MMR and Pertussis immunizations on their files which poses a potential health, safety or personal rights risk to persons in care.
