Personal Rights — Based on observations and interviews, Staff #1 did not provide child comfortable accomodations during a diaper change. Furthermore, staff #2 yelled at child #2 as a discipline methond to control the child's behavior, which posed an immediate risk to the health, safety, or personal rights to children in care.

Activities/Napping — Interviews, observation and record review revealed that on multiple occasions, during naptime, there was one teacher supervising up to 24 napping children, however, there were several children walking around the classroom. In addition, there were no remaining teachers immediately available to meet the overall ratio requirements. This poses a potential risk to the health, safety, or personal rights to children in care.
Responsibility for Providing Care and Supervision — Based on LPAs observations, the facility did not adequately maintain contastant supervison of children as they were observed running without shoes, muddy clothing and children playing in puddles. Furthermore children were observed climbing furniture and hanging from the play structure due to inadequate supervision, which poses/posed a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space — During LPAs' inspection of the outdoor activity space LPAs observed the concrete... and grass surfaces posed tripping hazards as the surfaces had potholes, cracks and unlevel areas, which poses/posed a potential health, safety or personal rights risk to persons in care.
Teacher-Child Ratio — Interviews and record review revealed that staff have been out of ratio on multiple occasions, typically during early morning, while coverage is being arranged. This poses a potential risk to the health, safety, or personal rights to children in care.
Buildings and Grounds — Interviews and LPA observations revealed that classroom floors, furniture, restrooms and outdoor play areas were not well maintained. In the “Twos” classroom, the floor areas around tables were stained and sticky. In the restroom, LPA observed dirt and grime on step stool and floor corners. Toilets were unflushed with urine stains on toilet seats. Toilet bases also showed exposed rusty bolts and grime. Toilet paper was scattered throughout the restroom. In the outdoors, children's socks were in the mud and several cleansing wipes were scattered on the ground.
Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above. LPA observed the slide on the play structure is not latched correctly into the metel poles, which poses a potential health, safety or personal rights risk to persons in care.
