Infant Care Food Service — Based on the information gathered from the self-reported unusual incident report, observations, and interviews with three staff, it was determined that S1 fed the wrong bottle of breast milk to Child #1 (C1) that belonged to Child #2 which poses a potential health, safety or personal rights risk to the persons in care.
Posting of License — Based on observation, the licensee did not comply with the section cited above in all posting requirements were not posted on the day of the inspection which poses/posed a potential health, safety or personal rights risk to persons in care.
Fixtures, Furniture, Equipment and Supplies — Based on observation, the licensee did not comply with the section cited above in both rooms Toddler A &B are bathrooms are used as storage. LPA observed large trash bins, brooms, and empty boxes, children in Toddler B use the bathroom sink to wash their hands which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Care Center Fixtures, Furniture, Equipment, and Supplies — Based on observation, the licensee did not comply with the section cited above in room Toddler B did not have a working sink next to the changing table, staff have to walk to the restroom which is about 6 feet away to use the sink that is in the bathroom which poses/posed a potential health, safety or personal rights risk to persons in care.
