Personnel Requirements — Based on record review and interview, the licensee did not comply with the section cited above in that Staff 5 (S5) does not have EMSA approved Pediatric CPR/First Aid training, even though they transport children alone. This poses/posed a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on record review, the licensee did not comply with the section cited above in that Child 3 (C3) & Child 6 (C6) did not have documentation of their immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.
Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above in 1 out of 5 children's files, which poses a potential health and safety risk to persons in care.
