Infant Care Teacher Qualifications and Duties — Based on the record review, the Licensee did not meet the above regulation which poses an immediate Health, Safety & Personal Rights risk to the children in care. During the staff file review the LPA was unable to review transcripts for S2 that included the required infant units. Licensee was unable to find proof of completion for required courses.

101238(c) Buildings and Grounds,All outdoor and indoor passageways, stairways, inclines, ramps, open porches and other areas of potential hazard shall be kept free of obstruction. This regulation was not met as evidenced by: Based on LPA observations, interview and by the licensee's own admission, the facility ramp used by the public to enter the infant center is in the advanced stages of deterioration which is an immediate risk to the health and safety of the children in care and the public.

Personal Rights: The licensee shall ensure that each child is accorded the following... To be accorded safe, healthful, and comfortable accommodations, furnishings and equipment to meet his/her needs. The requirement was not met as evidenced by: Based on interviews and documentation received, the licensee did not comply with the section cited above as children are changed on the floor without a changing pad and do not have comfortable and sanitary sleeping equipment. This poses an immediate health, safety, or personal rights risk to persons in care.

Outdoor Activity Space — Based on the observation, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the facility tour the LPA was informed the outdoor play area has not been utilized for approximately two weeks. Upon inspection, the equipment was observed to have webs, a collection of thick dust/dirt, jagged edged cement block wall.

Staff immunization requirements — Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the staff file review the LPA was unable to review required immunizations for S2. Required immunizations are MMR and DTap.

Mandated reporter training — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the staff file review the LPA was unable to review current Mandated Reporter certificates for S1 and S2.

Modifications to Infant Needs and Services Plan — Based on the record review, the Licensee did not meet the above regulation which poses potential health and safety risk to the children in care. During the children’s file review the LPA was unable to review a current Infant Needs & Service Plan for C2.

Food Service — Based on the observation, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the facility the LPA observed open bowls of cereal with milk & spoon in them, a bowl of watermelon, an open applesauce with a spoon in it and a sippy cup with fresh milk in it but a thick ring of unknown grim on the inside of the cup.

Infant Care General Sanitation — Based on the observation, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the facility tour the LPA observed indoor toys with caked dirt on the hard surfaces and caked on unknown substances on the soft toy attachments.

(a) The licensee shall be responsible for ensuring that children with obvious symptoms of illness including, but not limited to, fever or vomiting, are not accepted. Based on the interview, the Licensee did not meet the above regulation which poses a potential Health, risk to the children in care. During interviews it was disclosed a child was accepted by the facility with knowledge he/she had vomited prior to drop off. Additionally, during the child was at the facility the child vomited two more times.

Infant Needs and Services Plan — Based on record review, the licensee did not comply with the section cited above in 2 out of 2 infant files which poses a potential health and safety risk to persons in care.

Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above in 2 of 3 infants records which poses a potential health and safety risk to persons in care.

Immunizations — Based on record review, the licensee did not comply with the section cited above in 1 out of 3 infant files, which poses a potential health, safety risk to persons in care.

Indoor Activity Space for Infants: Indoor activity space for infants shall be physically separate from space used by children in the child care center and school-age child care center components. Based on document review and interview, the licensee did not comply with the section cited above. Infants were relocated to a preschool classroom without Department authorization/waiver. This poses/posed a potential health, safety, or personal rights risk to persons in care.

Infant Care Center Sleeping Equipment — an infant was observed with a pacifier that has a clip/ribbon that was attached to their shirt. This poses/posed a potential health, safety or personal rights risk to persons in care.
