Personnel Requirements — Based on interview and record review, the licensee did not comply with the section cited above in that 1 out of 3 staff files reviewed, Staff 2 (S2) did not have a health-screening report available to review. This poses/posed a potential health, safety or personal rights risk to persons in care.
Reporting Requirements — Based on observation, interview, and record review, the licensee did not comply with the section cited above in that there is construction being completed on the children's playground starting 6/8/26 and ending in approximately 3 weeks. This structural change that temporarily reduces the outdoor activity space, was not reported to the Department prior to making the change. This poses/posed a potential health, safety or personal rights risk to persons in care.
Administration of medication to a child — Based on record review and interview the licensee did not comply with the section cited above in that 1 child has inhaled medication that staff may need to administer. However, the facility staff present do not have written instructions or contact information as noted on the LIC9166 form. This poses/posed a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on record review, the licensee did not comply with the section cited above in that 2 out of 5 children's files (Child 3 [C3] and Child 4 [C4]) did not have a completed LIC 613A (9/96), which poses/posed a potential health, safety or personal rights risk to persons in care.
