Mandated reporter training — Based on observation, interview, and record reviews, the licensee did not comply with the section cited above. At 1:15 pm, during staff file reviews and interview, 5 out of 5 stafff members did not have their current mandated reporter training certificate, which poses a potential health, safety or personal rights risk to persons in care.
HSC 1596.841 Current roster. Each child day care facility shall maintain a current roster of children... The licensee did not comply with the regulation above as evidenced by: Based on records review and interviews, the director did not have a current roster of children in care during the unannounced visit. The director has class rosters without the required information that would appear in the LIC9040.
