During staff file review the LPA was unable to review an LIC 503 Health Screening or tuberculosis screening for S1 and S5. Director stated she cleared these same deficiencies for the same staff last inspections, however, she was unable to locate the LIC 503 during this inspection.
Child's Medical Assessments — Based on the record review, the Licensee did not meet the above regulation which poses a potential safety and health risk to the children in care. During the children’s file review the LPA was unable to review a Physician’s Report for C3.
Immunizations — Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the children’s file review the LPA was unable to review immunizations for C3 and C6.

During the children’s file review the LPA was unable to review an Admissions Agreement or proof the Authorized Representative(s) received a copy for C3 and C6.
