Criminal Record Clearance — Based on observation, record review, and interview, S11's criminal record clearance was not transfered to the facility prior to the first day of employment at this facility, which poses a potential health and safety risk to persons in care.
Responsibility for Providing Care and Supervision — Based on interviews with staff (LS, S1-S3) and parent (P1) conducted between 10/20/25 and 12/01/25, it has been determined that facility staff failed to provide adequate supervision resulting in C1 choking C2, which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on record review,staff ( S1,S2, S3, S4,S5, S7, S8) were missing proof of immunity or vacination against measles and/or pertusis, and/or influenza the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on record review, staff ( S1, S2, S5, S12) were missing evidence of negative TB results. During record review S1 and S12 were missing LIC 503 health screening report. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Records — Based on records reviewed all staff were missing LIC 501 the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.
Child's Records — Based on records reviewed all children's files were missing a copy of admission agreement., the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Sign In and Sign Out — Based on records reviewed there were more than five instances where guardians were not signing children in and/or out. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Limitations on Capacity and Ambulatory Status — Based on observation the center has a toddler component (18 months-36 months) in the "Explorers" classroom, LPAs observed at least three children (C1, C2, and C4) over 36 months old co-mingling in the toddler classroom.the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
