Teacher-Child Ratio — Based on observation and record review the licensee did not comply with the section cited above. 19 children with one teacher and an aid, which posed an immediate health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above because the floors of the classroom, bathroom, and kitchen were not clean. Additionally, the kitchen and toilets were unsanitary, which posed a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on observation, the licensee did not comply with the section cited above because the solid waste container in the classroom does not have a cover that is kept on which posed a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review and interview, the licensee did not comply with the section cited above because both the director and S1 do not have valid mandated reporter training certificates which poses a potential health, safety or personal rights risk to persons in care.

Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above because 3 of 6 children's documents LPA reviwed do not have LIC 701 Physician's reports which poses a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on observation, the licensee did not comply with the section cited above because one child (C1) was unattended for two minutes after leaving the outdoor play area and entering the classroom, where the LPAs were located. C1 played alone for two minutes in the classroom. During this time, the director was supervising the other children in the outdoor play area, and the aide teacher was preparing lunch in the kitchen. After two minutes, the director entered the classroom and found the child which poses/posed a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation the licensee did not comply with the section cited above. LPA observed trash cups were scattered and the door of the storage room was broken in the playground, which posed a potential ealth, safety or personal rights risk to persons in care.

Reporting Requirements — Based on record review and interview LPA was unable to locate the UIR the director referred in the system and the director was unable to provide a proof of the UIR submission, which posed a potential ealth, safety or personal rights risk to persons in care.

Drinking water lead testing — Based on record review, the licensee did not comply with the section cited above. The facility did not condcuat water lead test, which poses/posed a potential health, safety or personal rights risk to persons in care.

Indoor Activity Space — Based on observation, the licensee did not comply with the section cited above. Toilet paper and paper towels were observed on the floor in the restroom,] which poses/posed a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on record review, the licensee did not comply with the section cited above. 3 of 6 toilets were broken in the restroom, which poses/posed a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above. The facility did not maintain the above immunication records in S2's files, which poses/posed a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review, the licensee did not comply with the section cited above. The facility did not maintain the TB test result for S2, which poses/posed a potential health, safety or personal rights risk to persons in care.
