Reporting Requirements — Based on interview, observation, and record review, the licensee did not comply with the section cited above in that the licensee changed which rooms were being used for licensed child care, and did not inform the Department of this change, which poses/posed a potential health, safety or personal rights risk to persons in care.
Limitations on Capacity and Ambulatory Status — Based on observation and interview, the licensee did not comply with the section cited above in that 15 children were present on the playground at the same time. There were only 5 children signed in for the day. All of the children were in the same age group. However, both the school district children and the licensed preschool children were using the playground at the same time, which poses/posed a potential health, safety or personal rights risk to persons in care.
