Reporting Requirements — Based on interview and record review, the facility did not comply with the section cited above in that LPAs observed the director listed on the LIC200A, Melissa Williamson, is no longer the current director and the department was not notified of who is currently the new director which poses a potential health, safety or personal rights risk to persons in care.
